St. Joseph Health Clinical Program Assistant in Lubbock, Texas

Job Summary: The CHP Care Coordination Assistant will serve as a department representative providing service and quality simultaneously. In addition, the Care Coordination Assistant will assist the CHP Assistant Manager and Nurse Manager as needed. Overall, this position will be responsible for assistance with patient referrals, intake, outreach and other administrative duties as assigned. The Care Coordination Assistant will demonstrate efficiency in performance of duties to ensure efficient operation of the department. The Care Coordination Assistant is expected to have critical thinking, communication, problem solving and decision making skills in order to contribute to a positive care and work environment.Essential Functions:1. Serves as the support person for all intake and referrals (including daily overflow or fill in during absence of the Care Coordination Liaison).2. Record minutes for department or any other types of meetings. Assist with preparation for meetings as necessary.3. Review and track mileage for all staff through Concur reports.4. Enter time and attendance in Kronos for Care Coordination staff.5. Assist Care Coordination Liaison with maintenance of accurate documentation to ensure all referrals are processed in a timely manner.6. Responsible for ordering/maintaining supplies for staff and patients.7. May present program to patient and family at bedside Conduct brief patient interview to provide additional information to care coordination team in preparation for the initial visit8. Assists team with outreach calls to new patients. Responsible for scheduling initial appointment in settings such as the patient’s home, physician’s clinic or on-site navigator consultation rooms.Minimum Position Qualifications:Education: High school diploma or GEDExperience: Customer service or prior health care experienceTraining: Expertise with Microsoft Office products and general technology Preferred Position Qualifications:1.
